I'm surprise that you can stay with one character for IG hundred of years. How can you even survive for that long as a character? My human characters tend to die just after they reach 20 - 30 years old.
Do we actually have to simulate time spent in the village to get kids, or can we just go out adventuring all day? Do we ever have to talk to our spouses, or spend time in the same map with them?
As for whether you have to be in village while simulating time, I'm not sure (but would assume so). I haven't had a chance to try everything out yet.
