How essential are 18+ scenes?

I heard that this version doesn't have the 18+ so I was wondering if the 18+ scenes are important to the game or they don't add much to the overall experience?

I would say that such scenes aren't essential in this case because they feel very 'tacked on' and don't fit the narrative. I've also read a number of interviews and articles where their presence has been discussed and when the subject comes up the explanation for why they are in the game seems to change: sometimes they were supposedly 'planned all along', and at other times they were added just to boost sales.

In my opinion, Fate has ultimately always been about the love between characters and their respective routes. Those sex scenes, no matter how cringey people say they are, help further develop the relationships between characters.

This question is a matter of debate. Essential is a strong word. Does the game work without the scenes? Yes, mostly. Is it better without them? Some say yes, I tend to disagree. Not that the scenes themselves are well written, the descriptions are kind of laughable at times, but I never quite thought they were the brick walls that many do.

I also thought the replacement scenes were meh, at best and generally felt like fairly hasty replacements in and of themselves. Personally I wish the game would just have cut to black so to speak in the RN versions, implying the sex, or summarizing it, which I think would have worked better.

And then of course there is Heaven's Feel, where the sex is a pretty huge part of the story. The toned down version just doesn't hit as hard when it comes to the big plot reveals, and that route is where I think they are the most essential. Even the movie version of Heaven's Feel is more hardcore than the censored game release from what I recall.

Is the game still worth it without these scenes? Yes. Is cutting them as big a change as some other VNs? No. Do I hope this version gets a fan patch sooner rather than later so people who want the original experience can play it. Definitely yes.

In my opinion, the scenes in the first two routes were all very forced and the logic for why they happen is deeply laughable. If the characters just said "I like you, let's have sex" it would've been better, but instead they create silly contrivances on why exchanging sexual fluids is a method of magecraft. That means you'll be reading through a fight or a tense scene only for the story to go "the only thing that can save us now is banging!" which, you know, at least to me it read like a very silly and out of place porn parody. Plus, the act itself is not written as a "show of love and intimacy" between the parties involved, it is porn through and through, meaning it caters to obvious male fetishes and is not actually trying to contribute to a love story.

That said, as others have stated, Heaven's Feel, the final route, is the only one where said scenes are actually contextualized and do serve a larger purpose to the plot and characterization. One of them being included in the movies is a valid artistic choice because of that (and even then, it's a scene that's reframed to be very much about the intimacy and plot relevant details and not explicit). In Realta Nua, the context surrounding the scenes is implied, so you won't lose crucial information in any shape or form, but I can concede there is an argument to be made for the original rendition being better in that specific case.

Fate novel is way more focused on the romance, compared to the anime adaptation, nonetheless I celebrate people enjoying it with or without patch and I find a bit cringy to see people proclaiming that the novel is way better one way or another in so many posts

Nah not really. Though I will say some scenes I prefer them specifically for the HF route and in a few other spots in the game.

They were only in the original to drive sales, but they're so badly written it's hilarious. Them not being there makes the story better, but in Heaven's Feel they make a lot more sense to have. Typically the integration is horrid and interrupts the flow of the story, but they don't really hurt to not have.

Actually, we're spending over 50 hours on a romance visual novel, and we want the characters to deepen their relationship and eventually have sex—yes. It's an adult visual novel, and there's nothing wrong with sex between a couple, especially when it's pretty important in the third route, lol. I’m not sure how many romance books you've read, but let me tell you, the ones that don’t have characters consummate their relationship usually leave readers feeling unsatisfied, especially those who want to see a deep development between the characters.
