PSA: A brief history of this game's different verions
A lot of people asking about the 18+ version with H-scenes don't seem to understand how this game's release history worked in Japan. So I'm going to go over that real quick.

2004: The original PC release of Fate/stay night. This is the ONLY version of the game that contains H-scenes. It also lacks voice acting and does not contain an animated intro sequence for the final route. The two for the other routes use the song This Illusion.

2007: The PS2 release of Realta Nua. This version replaced the H-scenes, toned down some of the more explicit dialogue, and added voice acting and the epilogue. New opening animations were created for each route using the song Ougon no Kagayaki.

2012: The Vita release of Realta Nua. This one is essentially the same as the PS2 version, but has new opening animations for each route, using the songs Arcadia, Horizon, and Another Heaven.

2015: The mobile port of Realta Nua. I THINK these are based on the Vita version, but with each route sold separately.

The original 2004 version was rereleased only once in 2006 (prior to Realta Nua). So for 17 years now, Realta Nua has been the official version of the game.

If you have ever seen any version of the game that contains both voice acting and H-scenes, it was a fanmade patch. Type-Moon doesn't seem to have any interest in rereleasing the original version of the game, so don't expect to ever see those scenes in any official capacity.

There was some major tampering, years ago. People took Realta Nua and slapped on it:
1) English fan-made translation to all texts
2) Re-added 18+ scenes, as a mod, together with the option to read the hardcore or the censored version at will
3) The voice acting from Realta Nua
4) The choice to change OST (original or Realta Nua's) at will
5) All scenes, animations, pictures/collectibles from both versions

Only Realta Nua vanilla is censored, together with Realta Nua subsequent releases. The censor begun for PS2 release first and remained a thorn, everafter.

Tsukihime: A Piece of Blue Glass Moon should be looked at as a totally different game from the original. It's a true remake rather than a remaster. Takes place in the 2010s instead of 2001, has new characters, changes some of the lore, all new art and music, etc. If you're specifically referring to H-scenes, no those are not present. But the this version of the story was never written with them in mind to begin with.

cool thanks for the heads up.
guess i should dust off my old versions of both tuskihime and fsn and reburn them onto new cds to stave off the degradation of age.
