Achievements straight up DO NOT WORK

I've never been so frustrated spending SO MUCH time working towards getting 100% in a game only to have NOTHING happen once I meet the requirements for a trophy.

I am not having trouble finding things, I am not having trouble with getting stuck, and I am NOT confused about what to do.

I have 35 hours into the game, and here's some specifics..

I have the fully upgraded extractor achievement, but not the achievement for leaving the entrance without a sword. THIS IS NOT POSSIBLE. YOU CANNOT get the nimble trimmer without leaving the entrance with no sword. When I just try to leave again with no sword, NOTHING HAPPENS.

This is definitely not the only achievement that's broken either. I have all 7 shaman linked, every save point linked, every node linked, every vista read, and the entire map uncovered, but i don't have credit for doing ANY of it.

I'm currently sitting at 15 of 31 trophies earned when I have DEFINITELY finished more.

There needs to be some way to view what's left to do in order to earn certain trophies so I can figure out what's going on... but even if there was, it would look like I finished everything required.

Not to mention, zero trophies / achievements for locking all cortex memories or fully connecting the entire living network????

You shouldn't be able to claim 100% in a game without doing the latter. Easily 2/3 of my 35 hours of play time was spent connecting everything to the network.... and once you actually do it... you get absolutely nothing at all while people who skipped the most time consuming and complex aspect of the game get to claim 100%??? Stupid. Extremely stupid.
