Guide on how to make potions and which types?

I hear GASH really helps as it creates 'blood vials' which 2 of are needed to BREW one potion. You can also get by random drops. Also which herbs create what potion type?

You need
1) 1 Empty potion glass (just drink a healing one to get the empty)
2) 2 Vials of blood (Dedicate to 'Lady' to make into 1 Vial needed)
3) Click on empty potion glass - say yes to brew
4) Choose a herb type
5) Potion brewed (Warning: herbs maybe randomized every new game)

There isn't a list in the game (notes added for it might be nice) but it's built to help you while experimenting. When you use an empty flask it gives you a list of which herbs you can choose from. If you've used an herb before it'll tell you what that herb will make, next time.

I don't use too many potions outside of healing (though I really should). I can't remember the normal heal potion but large is made from Widow's Sap.

Also, if you dedicate yourself to the Lady, the cost becomes one blood vial.

So while making potions and trying to get the achievement for making all 21 potions, I wrote down which herbs transform into which potions and their effect, having the skill to hit ennemies and cause bleed from the perception branch is godly because you'll always get blood phial if the ennemy can bleed.

As for the herbs, potions and their effect, here it is (spoilers alert)

blood-pod -> antidote -> cure poison
silver-nectar -> spore-water -> inflict rot
holy seed - blindness
white needle -> bitterleaf brew -> cure madness
grave-leaf -> potion of poison -> poison
dream-needle -> blackwater draft -> remove torpor
silver oil -> sightsalve -> cure blindness
bone leaf -> phosphor -> explosive flask
widow berry - cure silence
grave-root -> sera-leaf oil -> cure rot
angel cap -> amberbane lamp oil
moon-rot -> torpor - cause torpor
blood-bane -> large potion of healing
moon-oil -> potion of healing
brittle nectar -> potion of stunning -> cause stun
blood needle -> acitra nectar -> cure stunned
bitter-bane -> water -> cure flaming
bitter-wort -> potion of silence -> cause silence
harrow-sap -> attuner -> cure restrict gifts from gods

Hmm, there seems to be a discrepancy between our notes, Nuage. I pretty sure when I used Moon-oil I got a Potion of Madness.

I found a Brittle-berry, but fouled up my attempt at brewing with it, and I hadn't autosaved recently, so I'm not sure what it does, and I've been having lousy luck at finding another.

Also, I have Sera Leaf in my inventory, but it doesn't show up in the list when I go to brew potions. I assume it's supposed to be an ingredient for Sera-leaf oil. A bug maybe?

Hmm the herb= type of potion could be randomized? i checked the submitted list to what i tested and only one matched. So All players - just test the herb/potion to see if the list matches the outcome.
