Turn off all mods and try again, load 1 mod at the time and see if one of them is the cause and if so contact the modder with the problem.
If the problem still occure with all mods turned off, verify

Not really. I have seen a mod which adds an event chain in which you can pick a color out of many options. But all options have to be predefined by the mod developer.

's such things as balance mods and mods which will cater to what you find fun.
Specifically for balancing, I think there's a really famous one that starts with A or maybe a T? or something? I can't find it

You can't earn achievements in Ironman either if you have mods turned on.

Only way to earn achievements in ironman is if you have no mods turned on.
