If you have dynamic provinces names on it might do that on it's own already but I am not sure what your culture would tag it as if anything.
EDIT: Apparently Latin culture group only has a name for Kaffa being Cafa instead so I don't think it would change the name.
Danzig for example will be called Gdánsk if taken by Poland.
With that said I'm not sure if Italy has this included because they're.. well.. Italy. But it might. Make sure the province naming is turned on next time you load the game by going in the settings menu before you press Play :)
If Byzantium still has any cores you could release them as a vassal and then feed them the land they need, but doing so would make them extremely powerful and they'd require an absolute ton of diplomatic power to reintegrate.
