If not the Holy Roman Emperor or an Elector:
Leave the HRE
Emperor receives event "Italy formed and left Empire"
Emperor riceves the opinion modifier -150 "Left the Holy Roman Empire".
Would be even possible as elector. Okay it's easier to become emperor than an elector. :D Without the -150 you could even try to join the hre again.
The link below is patch 1.12 with Common Sense enabled. The link will take you to the part where he is about to form Italy, he is already Emporer, and then he will put his provinces back into the HRE as Italy.

EDIT: if you watch the entire LP, he became emporer merely by increasing reputation with electors, none of them were Vassalized.
