Milan PU now subjugation, why?

You'll be able to integrate them quicker and move on elsewhere. The only offset for that is the increased Liberty Desire based on their strength and whatever other non-PU vassals you have, but if you've managed to subjugate Milan and eat up the Aggressive Expansion for doing so, you won't really be worried about them being rebellious.

OK, so what exactly was changed then? Because a Force PU and a Force Vassal casus bellum both naturally have the same AE cost. And it would appear, from the patch notes, that the latter option is given when Milan is ineligible for a PU (ie. has become a republic), and therefore allows you to still take the nation over. It's worth pointing out that Milan CAN become a monarchy whilst still being in the disaster, and therefore IS eligible to become a PU.
