Achivements disabled for all current saves

Has there been a small patch recently? Literally all saves borked, classic

The last patch was on the 23rd November 2022, But a patch never disables achievements(but some major patches break your older save).

Are achievements enabled if you start a new ironman save?

Have you already loaded your saves and seen a disabled achievement icon or did you get warning and didn't actually load them? If you loaded the saves, achievements are permantently disabled for them and there is nothing you can do(unless you have other saves or self-created backups of your files which you have not loaded yet). If you aborted the loading due to the warning, can you post a screenshot of that warning message?

Did you buy some DLC? It will disable achieves if you bought DLC and then load the game, you get a message warning you.

It’s odd , it keeps happening on and off with Norway save. Seems to be ok for now but had to do a clean install. Can connection issues cause this also ?

I remember some youtuber saying that his achievement icon is grayed out because he has connection issues.

If steam is in offline mod, the achievement icon will be greyed out, but it is only temporary and it will be enabled again if you continue the campaign in online mode. I think it will register achievements which you got while being offline, but I'm not sure about that(it might only apply to achievements for which you still meet the conditions).
But intermediate connection problems while steam is in online mode can cause eu4 to permanently disable achievements. If you have an unstable internet connection, it might help to play offline. In any case I would recommend that you regularly check if achievements are still enabled and that you regularly create additional copies of your ironman saves so that you have something to which you can go back in case achievements get disabled.
