While it is weird to name your kids the same name it did happen back then. Just as Maria Theresa's daughters. They wont gain regnal numbers until they become king so it is Akiaos I and Prince Akiaos. When the older brother dies he will turn to Akiaos II
I know of no way aside from editing the save file but only if it is non-ironman
I dont know how to do it per se with EU4 but I would assume it would be near the same to do with CKII which I have done plenty of times before. See if you can resave you game and uncheck a box that says compress, download c++ notepad, go to your paradox interative folder, your eu4 folder, save game folder, rightclick the saved game and open with c++ then type in Akiaos to the search bar and go until you find the heir and change his name. Thats what I would do in CKII
You will have to look at the clues around it such as birthdates or titles or wifes name, etc.
