1) Any (not endgame tag) nation -> Prussia -> Poland -> PLC
2) Brandenburg -> Poland -> PLC
3) Poland -> Prussia -> Poland* ->PLC
*Every nation can enact a "Form XYZ" only once but as you start as Poland you can (re)form Poland later. (Poland -> Prussia -> Poland -> Prussia is not possible because forming Prussia twice)
No you cant, commonwealth is a endgame tag. Youd need do it before you form it.
Older versions of the game had no end game tags.
You could do stuff like mayan japan with prussian government and keep all of it as germany or the HRE.
