When the struggle event fires you can either resign and become an oligarchic republic or wage war on the nobility.
If you go against the nobles a huge number of rebels will spawn. Defeat them all and you get an event that changes you to despotic monarchy.
Note that you need to change to administrative for the cheevo.
So going republic is the only way to get that achievement?
Okay, so is there 3 options in total?
1. Keep sejm
2. Become Republic
3. Declare war on nobility

No, I meant administrative monarchy.
No, check the achievement requirement in the game interface. You need to upgrade to administrative monarchy. Which is as simple as spending 100ADM, but many people miss it since government forms are a bit of an obscure mechanic atm.
Do I need a male leader or do I have to change to another government type??

Just double checked the wiki, the last step is to change from despotic to Administrative Monarchy for achievement.
