What country are you playing? If you're playing England at the 1444 start, it's a PITA to get an heir even with everything going for you. Some leaders have a trait that makes getting an heir more difficult as well.

Royal marriages increase the chance of an heir being born, there are also ideas that increase the chance. Most of them are NIs and thus unique to specific nations, but they is an idea in the Influence Idea group that increase heir spawn chance: Establish Cadet Branches.
Religion can also increase chance of a new heir; such as Sunni Islam.

If you wanted to cheat, you could use the "add_heir" console command. I believe that's the command, anyway.

i have been known to cheat and add an heir with the console , you can also set there age with a command.

it's not cheating , its righting an injustice :)
