Once the heretic enemy nation is down to a single province containing the CoR (or even possibly 2 provinces if the CoR in located in the capitol), then simply force them to change religion in the final war. The capitol province immediately converts and the CoR simply disappears.
The key is that the CoR MUST be located in the capitol province to get force religion to work.
Alternatively, you can capture the CoR province and use religion ideas plus the advisor and the mission choices to get additional conversion percentages. Eventually, with enough patience, you will be able to overcome the 5% malus and convert it.
just check every few years incase a new centre has formed you only really need to take land if they are to big to force convert

Ive just united the HRE in my current game (5 time total) having destroyed 3 centres of reformation and converting all but 5 back to catholic preventing the league from every starting

War force religion and if you have the diplo slots free Mil access if you expect they cant cope with rebels - or start another war elsewhere for access if you need to go helkp them out
If the CoR is not in the capitol, force religion pretty much does no good since they will eventually just convert back with the Center or by rebels. For small countries, they might be able to stay Catholic by converting all their provinces. Even then, a nearby CoR might just eventually change them back.
Getting rid of the CoRs is always the top priority to make it fully permanent.
Also, the nation has to be low enough development to annex, as the peace treaty costs for annexation, vassalization, and force religion are all the same base value.
Therefore, a large nation like Bohemia will require several wars to beat them down to size to eventually have them really change their religious ways permanently.
