leaving hurts your relations with austria but the alliance should hold.
staying in the hre hurts your relations with the other italian states and gives - 2 or 3 unrest.
I know that you take a relations penalty with Austria if you choose to leave but I don't know if this would necessarily break the alliance. If your relations are high enough, you may be able to keep them as an ally. To be safe, it might be a good idea to gain an alliance with somebody like France or Castille before leaving if you want protection.
