From what I can see being inferred by the developer, Pseudoregalia is basically one instance of probably countless interactions Sybil has had with people.
I don't think a sequel may ever happen, I think I am content with this game alone tbh. I think it gives all the ideas it needs to narratively without it having to explain further and it accomplishes all it needs to mechanically. What we could use is an update that could add in some more lore tidbits in those unreadable notes that are present. And fix a couple annoying bugs.
From how I see it, I'd approach this like a FromSoftware narrative where they give you all the pieces needed to solve the narrative but they don't blatantly tell you. For example, the bleeding heart outfit along with her comment on how "revenge sounds nice" I think kinda gives the idea that maybe, Sybil was once in a dark place mentally in her own life and now has since dedicated hers to helping others in similar situations since she knows that pain. With this alone I don't think a prequel is needed either on who Sybil is.
Makes me think that she could have been a dreamer that became trapped in the dream world, either by her own hand or by the actions of another. With that latter bit would explain why the idea of revenge sounds appealing to her. Either way, because of her state, she drifts between dream worlds and occasionally help out fellow dreamers with their issues, or just knock them out of their dream world that they are reluctant to let go of.
Either way, if there's a sequel, I doubt it'd have the same title as this game considering 'Pseudoregalia' effectively means 'like royalty' and relates to the theme of that particular dream world and the form the dreamer took on as a princess.
Tbh to add to that, there is the hand enemy that looks like the pointing finger of a parent,
