what happens if I refuse Navlaan quest to kill Cale?
Does it skip to the next target?

No. You'll be given the opportunity to change your mind. Until then you cannot progress.

Feel free to kill Cale, he's an entirely unimportant NPC despite a small bit of lore info. If you don't want to kill him, or the other NPCs; but still wish to complete Navlaan's quest, there are ways to go about it.

yeah. For Cale, I actually need to complete the game. Meh, Still a ways off, but it can wait.

Nothing, saying no doesn't do anything. You can get his item after finishing the game. Or you can just kill him since he is useless.

i don't know if it was because of not exhausting his dialogue (i have tried to be sure), but i got this achievement after talking to him after killing nashandra even if i had all bonfires lit

Nashandra is the last boss you need to kill in order to lit the map
