Game is CPU-bound, for once, it seems.
Two things, you can try using GeDoSaTo mod to lower in-game rendering (instead of raising it) or you can get a better machine. Your options really are limited and only the latter one I mentioned is a definite solution.
You are also using integrated graphics on a laptop, possibly Intel HD 4000, which is not terrible, but also not ideal. I have no idea if Hd4000 is good enough for Dark Souls 2.
This issue is also happening in Dark Souls Prepare to Die in which the frame rate is so slow, its a slideshow in which my button presses have a delayed response.
Having a similar issue, but game use to run fine with my specs, and i've 0 idea how to fix my problem. Framerate stutters, and my character has had his limbs amputated with a hand floating over my head. I have updated the drivers of both graphic cards with 0 results.
Forgive me as I am new to computer gaming and hardly know ♥♥♥♥, (such as what my motherboard is and if I even listed it below), I simply bought the computer, monitior, extra fan, power supply, memory, and graphics cards, then had them assembled at Staples. All of this was purchased back in December of 2013

Any other solution beyond Driver Updates?
Edit: I have attempted to reinstall the game.
This issue is on all 3 dark souls games, where the settings are turned down and cant handle the frame rate on any of the 3, the 1st is literally a slide show with delayed button responses and I cant even get Dark Souls 3 to launch. All my other games are fine.
That does seem to have something to do with the issue. The moment I clicked the NVIDIA Control Panel, what I got was:
"You are not currently using a display attached to an NVIDIA GPU"
Not sure if this has to do with it, but I had just bought a new monitor. Previous one died.
Also I attempted your google search suggestion, and I noticed I am missing the power tab from Intel Graphics and Media Control Panel Advanced options. Should be under media, but the next tab is Options and Support.
