Left click= left hand
Right click= Right hand
Double click either for a power attack.
Hold shift+Right click to use items.
Hold Shift+Left click to use world interactables(Bonfires, doors, talking)
Hold control and left or right click to switch your weapon in either slot.
Hold Control and click the middle mouse button to two hand your right weapon.
Hold control and hold the middle mouse button to two hand your left weapon.
Hopefully those will help you out, it is a little irritating and you can edit it to what you like, but hopefully that assist ya! Gl!
Keyboard and mouse controls will need some setup and take some getting use to but can be used effectively if you're patient.
That said, controllers seem a bit more suited for the Soul's games than a keyboard and mouse, so if you would like to avoid the initial hassle and frustration (that come with setting up and getting used to K+M), consider getting an controller to make your experience a bit more pleasurable.

I played Demon Souls & Dark Souls on PS3 back when I was a console gamer (granted that wasn't too long ago, only a year or two ago) and since I converted to PC gaming I've adapted to the keyboard and the "asdw" way of gaming which is awesome. You get so much control.
After much deliberation of whether to get DS2 on PS3 or PC, I actually decided with the PC version.
Now I'm regretting it. Although the controls I have picked up and mastered after the first day of playing it, it is reaaaaaly irritating me that the on screen display shows everything as XBox controls, (ie: "Press A to pick up item")
I've never owned/played an Xbox, as I grew up from the PS/PSOne/PS2/PS3 and never really gave Xbox a chance when it came out. I have no idea what their controls resemble, nor do I really care.
Ultimately what I'm saying is that I'm playing the damn game on PC, I want to see the details relating to the PC, not some lazy knocked up PC version where everything is showing XBOx controls. Given this, I'm surprised they bothered to make subtitles in English.....
