Behold, borderless window, kinda
1. Set your settings in game to windowed mode and make the resolution match your monitor's native resolution.
2. Save these settings and exit out of the game.
3. Find the userconfig.properties file in <yourSteamAppsFolder>/common/Dark Souls II/Game/ and open it up with notepad.
4. You'll probably see only 2 settings (I did). If there are more than 2, you'll want to replace only these 2 most likely. Find:

App.Window.X = 0
App.Window.Y = 0

and change it to something like:

App.Window.X = -5
App.Window.Y = -32

5. Save the file, start up the game.

This shifts the default window position of the game to sit where it's "borderless." You might have to adjust those values to work for you, but these are perfect for my monitor.

I've always used a program called Shift Window to make games go into borderless window mode when they refused to do it themselves. and it worked just fine for this game too.

my native resolution is not even in the window mode selections
