You can even enable to once Dark Souls II goes into offline mode (game doesn't automatically put you online when the connection is established again; you have to exit to menu first).

Outbound rules,
Next,
Select the .exe to Dark Souls II,
and confirm that you are blocking this path in your new firewall rule.
The game will be 100% forced to start in offline mode as long as the Outbound rule exists.
