On topic, I don't know what could be wrong but you seem to not understand a lot about the game in the first place. There's too many thing that would make enemies stronger or seem stronger.
Your weapons could be broken, you could have accidently used an ascetic, summoning shades and phantoms raises the HP of enemies and bosses. A ring giving you bonus damage ie: ring of blades could have broken or you removed a ring granting similar bonus's. Wielding a weapon single or double handed would affect damage too. Imbuing weapons with certain elements like your poison would lower base damage in half but allow you to poison proc, but it would be useless on a hammer like I said. elemetals work the same way, and some enemies are stronger against certain types of damage. In Iron keep they're weak to lightening, but resistant to fire and poison.
There's undoubtedly more but I'm out of ideas for now.

Actually I'm usually stoned when I play Dark Souls 2 and find it easier and less aggrivating when no ♥♥♥♥♥ are given.
