Great Soul Embraced - but can I be sure?
Killed the Duke's Dear Freja in Brightstone Cove Tseldora. Received the spider's soul in my inventory, but the actual Great Soul drops from the dragon in the form of a glowing red orb on the ground. Approached to grab it and did so, but I was careless and disregarded a surviving spider. It attacked and killed me the exact moment I picked up the red soul. Then I died on my way back, defeated by gravity. Once I finally got back, there was no soul and instead the dragon crystal was in its place.

I did get the "Great Soul Embraced" message right as I picked it up, but I'm getting a little paranoid if the game actually "knows" I did. I noticed for example that if you receive a fatal wound right as you kill an enemy (or maybe even a split second after), the souls from that kill don't get added to the bloodstain. Fear of the possibility that the red soul similarly wasn't counted is admittedly hindering my immersion at the moment.

Emerald Herald gave me the comment she should after I have 3 grand souls (good sign, as I already killed Rotten and Lost Sinner before that, but not the Old Iron King yet). The doors through Shrine of Winter are already open (guess I must have already had the required soul memory when I reached it). Are there any other spots in dialogue/gameplay where it will matter if the game actually properly registered me picking up the red soul?

No there is no any other spots like that. In fact, u can even end the game without getting all great souls and nothing will change.

In these scenarios the enemy that u have killed doesnt realy die. I have done some experiments with killing bosses as a phantom after death of the host. In this cases boss doesnt actually die
the fog gate appears again and the boss battle should be fought again.

as above

for gameplay it's all fine
as soon as freya dies befroe you it's ticked of to be dead
if you die after her, it remains ticked off

if you die before picking up her "great soul"( the red thing on the floor ) they remain there ( it hink )
if you die after you picked it up it's registerd

the "great soul" isn't a physical item or entity
it's no more then a marker for gameprogression.. if it displays the requirements are met and it is registered

the boss souls is a boss soul and is conciderd a physical item which you can lose ( although you have to mnaually delete it to do so :p ) -- this is separate form the "great soul - concept"

and the concept of "great soul" is a lie anyway
it's there for estetics
the marker itself is the primal bonfire after every major boss

if those are lit ( regardless of when you do it ) a flame pops up in majula to visualize confirmation ( and more estetics )
the shrine door activates after all 4 are lit... nor more no less ( regardless of whatever lore-estetics are given )
they put a failsafe in in case stuff like this happens - you can't mess up a primal bonfire

either it is lit or it is not :3 ( there is no middle way )

other then that... gameprogression can alos be achieved by collecting 1.000.000 souls in total
( generally you have to farm for this... normal gameplay doesn't provide enough )

the 4 bosses as under normal gameplay is the faster way

using bonfire ascetics on the rotten is the strategical fastest way to ramp up to a million souls

Makes me wonder, actually. If a player does indeed ignore/forget the red glowing orb, what would he find if he revisits the boss area at a later time - still the orb, or already the dragon crystal. Would be one of those things I'd be eager to personally check - if it was a different game, specifically one with a reload button.

I'm curious if non-boss enemies killed by this double-KO count towards the counter before they stop spawning - or does the counter actually check for times you **collected souls** from the killed enemy instead, acting as a failsafe.
