updated to the newest nvidia drivers yesterday, this is ridiculous

Wow, thats low, there must be system trouble somewhere because my game is flawless at 60 fps with adaptive vsync forced with NVInspector and i'm on a GTX580 with 2500k, so reasonably similar to your rig, you should be easily 10%-20% faster than me!
I have Ultra preset running, so everything maxed with FXAA.
Make sure you have the SSAA turned off as that ruins video cards, I really have no idea what card could run with that stuff enabled.

I get 60fps on 'Medium' settings but get 100% CPU usage constantly and roughly 50% GPU usage. 'High' gives 30fps at best
I think your problem is that it has chosen the integrated graphics on your processor. Go to Nvidia Control Panel -> Manage 3D Settings -> Add Blacklist_dx11.exe and then change "Use global setting (Auto-select: Integrated)" to High-performance NVIDIA processor"
This boosted my performance by miles.
Before this I had 30 FPS on low and after I changed it I got 75 fps with all maxed out.
