Changing Difficulty
I'm thinking of buying but read that you can't change difficulty in the first run.
Is this true

Yes , you can do it in any major city. There's a statue that on the mini map the symbol kinda looks like a saw blade.
there is only normal and hard at first, expert and penitent will unlock as you play. Torment 1, 2, 3 and 4 only become available when you reach level 60 and must be unlocked by completing the respective tiers of a challenge called "the pit"

Once you unlock expert and penitent they are unlocked for all characters, but i don't think you can just start off on them, you need gear and you out level your gear so fast those difficulties don't really makes sense until you stop getting stronger gear at lvl 60. They are more useful if you have a friend who can level you.

No there are only 2 from the start. After you completed the campaign the other 2 are unlocked.
There are two difficulties at start if you run a season char with campaign (Normal and Hard). This is true for all new campaign characters.

Expert is unlocked once you've reached some early point in the campaign and you have to change it at a statue in any major city. Expert is waaay to easy at the mid to end of the campaign, which spoils the fun.

Penitent difficulty is unlocked once the campaign is finished for that character, or if you skip the campaign at character creation I guess.

Torment difficulties have to be unlocked by completing the end game dungeon The Pit within 15 minutes at certain levels. Like Torment 1 at level 20.

You can change difficulty to a point. You get limited by the story. Once you get to torment though. In my opinion the level scaling doesn't feel as organic as it did in Diablo 3. I never feel like I get anywhere in this game.

In D3 I could pretty much pick any build in a season and feel improvement and work toward a cool as beastly build.

In this game, sure I can kill monsters fast and it's cool as hell bumping into other players but I just don't feel like I ever get anywhere.

The game seems designed to keep people playing as much as possible over them having as much fun as possible.
