You get in game rewards, ranging from armor, mounts, xp and stuff by playing the game and also logging in every day. That's the free pass. If you pay $20, you can get the Premium Pass, which gives you more of all that and makes it easier and faster to level up your pass and character. Its a very greedy and underhanded system that a lot of companies use now (mostly in F2P games), cause they put all these shinies in front of you and say, if you play for 20 weeks you can unlock this, or you can pay us $20 and unlock it in 6 weeks.
Essentially stuff that used to be free or seasonal is now paid or grindy.

This is acceptable for games like Fortnite etc that are free and actually give you something that at least can be seen by others en mass, unlike in Diablo where it also costs money.
