To ascend a character, you first need to level them to the absolute max you can. For 1 star heroes that means full abilities and level. from iso-8. For 2*+ that means maximum level champion. Basically they need to be leveled till you can't add another cover to them and can only use covers to swap skills around if possible.
Once you have your character at their absolute max level, you need a duplicate of that character in your roster. Then you need to take that duplicate to a required level. For 1*s that means maxed out (just like the first one you did). For 2*+ you need to get at least to champion so 13 abilities and promoted to champion.
When you have one copy of your hero at absolute maximum and another copy at maximum (1*) / champion (2*+), you can ascend the max level one by clicking the ascend button on their main page then, using the interface that opens, select the duplicate hero to sacrifice it.
That leaves you with your hero ascended at the increased * rating and at champion level. It also gains additional champion levels based on how many champion levels you had on the sacrificed character. It also leaves you with an open roster slot from the spot the sacrificed character was in.
If you're doing it to get the most rewards possible from each cover, you want to make sure you max champ your sacrificial characters before ascending. That way the covers you spend get rewards from championing the sacrificed character AND for the levels they give on the ascended character.

Champion's 2.0 Blog[d3go.com] on the dev site has the basic info on the system and the current "first batch" heroes listed aaaaallll the way at the bottom of the page.
There are also a LOT of comments that go through people calculating out the various cover costs / rewards strategies.
I got the link to that page from News and Announcements here in Steam forums and I expect there will be more official announcements when they add more heroes to the ascended list.
For example, a 2* wolverine is max level at 94 (13 covers and fully iso'ed). Then you need to reach level 144 (via 50x extra covers) to champion it. To ascend your 2*, you'll need another level 94 wolverine. Combining them gives you a level 145 *** version.
To ascend that wolverine, you'll need to raise its level with covers (2 per level) to level 266. You'll need to combine it with another *** wolverine to reach ****.
At ****, you'll need 3 cover per level and must reach level 370 before you can ascend that character to *****. You'll need to combine with a **** 2* wolverine. It's a very, very long journey.
Ascended heroes do not take up multiple roster slots. Of course, after a few times ascending the same hero, the roster space required to continue ascending them gets to be a bit big just because you can hit a point where you need multiple slots dedicated to earlier ascension versions. For example, if you're Ascending a 1* to 5*, when the ascension hero is at 4*, you'll need at least 1 slot for a 3* version (intended to sacrifice as 4*) plus at least 1 for a 2* version and at one point 2 for 1* versions.
So, at worst, you'll need 5 slots dedicated to ascending that 1 hero. However it's only temporary because you'll quickly max that 2nd 1* and start collapsing them as you ascend the sacrifices and feed them into the higher ascended sacrifices to feed into the main ascension target. Higher starting rarity hero's aren't as painful though because they don't have so many levels of ascension to do each tier.
