how does beserkers unique item unlock? i have 9/10 but im unsure how to unlock it. i've cleared floor 10 on 50% health sevral times and even cleared the game with her at 50% hp being very diligent to keep myself below half. how does this item unlock?

Check the monolith, there might be some prerequisite missing.

You need do it in one run, if you buy alot belts, it give you more space for maneuver, for was sure that you hold on less that 50% hp
