In his bio there's a little backstory that he was tortured by Stank Gum. Hard to say if he was disfigured before or not.

The Chumbucket in the game looks like he suffers from a birth defect. Scotty's personality and voice just reminded me so much of what the character might have been like before the Fall. So funny! He's got a good Youtube channel though! BLACKTHUMB! :MMV8:
