I did both of those steps and now the game lags in much rarer instances, probably when it has to load a ton of assets when going from one area to another, maybe. Improved by at least 95% anyway.

I only didn't use one of the option they suggest which was ShadowsQuality=1 I think with it 0 it's just doesn't look good. Keep 1

You really have to start using google.
I've over 20 hours of running the game on Ultra High with no issue at all!. I tried turning the graphics down to see if that would help, it didn't. Anyone else still have this issue after negating write permission for all users?
I have also tried the running offline, as administrator and turning off steam sync.

After every win10 update they may turn it on again or change to other place (used to be in the xbox app)
Also in the past Cortana used to also give problems.
I would suggest in Windows 10 create a Gamer user and fix that user to the best performance. Turn off everything. There are many videos about the subject. Here's a recent one
