Yep. The latter of which is equivalent to a level-up on easy or normal.
There is a quest related, try the stairs and soon you will understand

I actually found that, since taking damage from the spiked ball knocks your character up and back a little, I was able to re-attach myself to the rope, just by hitting 'up' at the right time. So even if you fail to dodge it, you should still be able to get past it. It is possible that it's changed since I tried it, though - it was quite a while ago.
And yeah, 'down' lets go.
