Since there is three different variants for each (skinny, normal, muscular), that means you'll need to edit 6 files for each gender if you want to be thorough.
I haven't tried removing it from the diffuse since I'm not that good with Photoshop but I have tried removing the underwear from the normal.dds previously when I was making a stealth suit but it didn't turn out the way I was expecting so I scrapped it.

For female models, there is already a mod. Only this one file (diffuse.dds) I do not get edited and then back to the original format.
Your stealthsuit is make from shekskin, right?
The last time I was messing with the diffuse and normal texture files, I noticed that the textures for Cannibals variants are slightly different from human ones. The normal body is the same I think but the muscular and skinny are completely different. The muscle definition is different.
Plus I also noticed that the resolution is different. The resolution for Cannibals are smaller so a lot of fine details are lost. It doesn't really matter for Cannibals I guess since they're covered in full bodypaint anyway.
I tried making a modified nude normal map textures (without the underwear lines) for my suit and when I tried to apply it to the human body it produced some weird shading effect.
I know the normal map for human skin are supposed to be saved in DXT5_NM in grayscale but it doesn't seems to be working for me.
I have downloaded that mod that you're referring to from Nexus but haven't tried it in game. I only wanted to see how different it is from mine and I got to say the diffuse looks pretty good but the normal maps looks like it was done in a rush. ;p
