Unlock the Prisoner Cage and have one of your guys be nearby. Exit the cage when noone is looking and pick up the Slave with the other guy and just carry him out of the city.
No guard will bother you.
Alternatively you could have left your guy a slave, taken the advantage of free room and board to increase his stealth and lock picking skills, then his toughness and martial arts in fighting off the guards when fully healed. Then broken out once you were reasonably well trained.

This can run the serious risk of losing a limb if you're not both careful and extremely patient with the slow-ass cage recovery speed.
If you're picked up by slavers, immediately disable 'obedient slave' and activate stealth while you're unconcious. As soon as you regain conciousness, they should drop you and there's a good chance they'll keep walking and leave you behind.
If you manage to get dragged across the entire god-forsaken map and put in a cell, you're in for some trouble, as slavers often have somebody with a crazy run speed on hand for runaways.
I tried to simply run away a few times which increased the toughness quite well. But I just didn't want to spend the time. The rest of my squad was camping out of town and I didn't want to wait there forever nor leave my guy behind.
Yes he has for now. After running away you stay an Escaped Slave for 8 hours and you have a minimum discovery chance of 10%. That is when you get seen (by a faction that takes slaves) there is a 10% chance they will discover his slavity and try to enslave that character again.
After 8 Hours the character gets the Ex-Slave Tag for 96 hours I believe (4 days) in which his chance can be as low as 1%. If he stays undiscovered all that time he loses his slavery tag all together.
As noted in the post above mine, it goes away after a while.
The funny thing is Slavery happens to be an easy way to train sneak, thievery and lockpicking without being considered committing a crime. The slavers never intentionally kill you, they always want to bandage you up and put you back in a cage to recover, so long as their A.I. isn't interrupted.
Depending on the camp, it can actually be very easy to escape. Just free as many other slaves as you can, and bolt out the front door with a group of them behind you. If you can run fast enough, and have enough freed slaves behind you, you should be able to evade most of the guards as they're distracted by the unfortunate people left behind.
If anyone else makes it out with you there's also a chance they will join your squad.
The hard part is not starving to death before you reach your next town, it helps if you are able to steal food the same night that you break people free. And then of course you need somewhere to lay low without starving to death. If you already have people outside that are not slaves, this can be a non-issue. Especially if you have any form of outpost already setup.
Is that 8 hours in-game or 8 hours IRL, to remove the Escape tag?
The Ex-Slave tag on my guy has finally run out and now he got a bounty on his head by the United Cities. 6500 Cats for Assault and Escaping Prison.
Atleast now I can deliver him to the police myself, get 6500 cats reward plus he will be released eventually.
EDIT: The sentence is 130 hours...so either leave that person behind and return later (or not) or use the time to fight stuff or mine ores.
So my guy was in prison already having served about half his time. Then I suddenly got a notice saying that my guy in prison was enslaved.
Well turns out a group of slavers came into prison and enslaved all the prisoners.
Typical United Cities bullcrap.
Well, the rest of my squad followed them out of town and laid a little ambush, crushing them into bloody pieces and now my guy is an escaped slave again...
