He looks like a slave
And then I am attacked, my portrait says' looks like a slave: - ' its greyed out. How do I stop looking like a slave?

but they sometimes say that no matter what. they just want to enslave you.

If you've got someone that escaped slavery, you'll have to give them clothes to look less like an escaped slave.
Giving them plastic surgery is also a very good idea, especially giving them back some hair.

There's also Manhunters that if they spot you, they'll just say that you look like an escaped slave, even if you don't, just as an excuse to attack you and sell you off to slavery.

Greyed out means you don't. Slave hunters will often try to enslave you anyway.

It's a bit odd because in the research bench greyed out mean you have the requirements (positive) but in the character screen it means you don't (negative).

It means that the slave hunters you ran into are using the excuse that you "look like a slave" to beat the snot out of you because you're a weakling on his own and enslave you for a quick buck because they can.

If the likes of manhunters attack you while asking their friends via floating text if you look like a slave they are just using it as an excuse.

You don't actually need to look like a slave/be an escaped slave for that to happen.
