I think you can see in the character inventory screen to which faction you are a slave to or hovering over the "Escaped Slave" label will let you know, plus how long more before you are fully free from slavery.
Edit:You can also try to disguise that Escape Slave through certain means such as fully covering the face, haircut and etc to lower the chance of getting discovered.
If you see any guard or anything saying something like "come here and let me look at you", run. That dialogue means they are on to you and there's a big chance they will see that you're an escaped slave.
And each time you are discovered as an escaped slave, the timer for Escaped Slave status is reset. So have the escaped slave lay low and stay out of cities and away from Slavers, Manhunters, etc. Have the other squadmembers do the shopping in town.
Your "Looks like a slave" meter was probably low hence the reason that prevented you from being recognised. Lowest you can go with the meter is 1% chance of being recognised. By not trying to lower the "Looks like a slave" meter and keeping it at 100% will always get you attacked.
really saves time if you don't feel like waiting for them to carry you to their base and escape there
if you are wellfed you can survive the entire slave-timer duration without needing to shop for food to avoid getting detected. getting slaved usually only happens to new characters so i just use that time for training other skills, maybe hoarding copper or something while sneaked so when im free i can cash in
there is a problem that persists even after your slave timer is gone and you become free again, the faction that enslaved you will sometimes recognize you regardless of being free/no slave, and they try to subdue you. so just watch out for the slaver faction and you can go into towns fine once free
Admittedly, plastic surgeons seem to be extremely hard to find. (They used to be in every bar.) But if you did know where to find one, you could have a party member carry your escaped slave into the bar for a makeover...
They didn't actually do anything after I unlocked their shackles, so I kept it up. Then I realized that they were following me (sneaking, as I was) - or that some of them were, at least. I got them out of town, thanks to some skimmers who drew off most of the guards, and we all ran west along the shore.
And yeah, pretty soon, as individuals, they either joined my faction or ran off, but then I had a bunch of starving ex-slaves to try to get to safety. That was fun! We made our way south through UC territory on the west, where the desert and badlands meet (into Sinkun, briefly, before that looked too dangerous to continue), through bands of manhunters, slavers, and assorted bandits.
I thought we were doomed when trying to get over the mountains into Holy Nation land, south of Okran's Fist. I've never seen so many skimmers in one place! Not to mention patrols of both sides in the war. (I did reload a saved game once, when skimmers completely wiped us out.)
I'd deliberately released some Shek from slavery (and I'd recruited a Hive drone in the UC), so we had to watch our step in Holy Nation territory, too. But that was simple compared to the effort it took just to get there!
And now, my slaves are only a few hours away from complete freedom, we're loaded up with building supplies looted from a ruin, and we're looking to found a settlement for freed slaves.
Wow, this was fun! Next time, I suppose I'll just try emancipating some Holy Nation slaves, but the Traders Guild in the UC really pissed me off (I tried stealing from their nobles, but... well, they've got a LOT of guards), and I'm glad my first attempt was such a challenge.
