how do you make a yard?
does it neeed walled off with doors and does it need a foundation? or can it just be out in the open?

can be anywhere, just mouse over the yard icon in the rooms tab and it will tell you the requirements

out in the open is fine, but you need to make sure your prison has walls around it....basically so prisoners cant just walk out before it lets you build it

My understanding was a yard still had to be behind it's own fence/door, not just an outdoor area within the fence boundaries of the prision. Am I wrong?

Not the best video, but you can see the Yard go from not OK to OK once the fences are added to the outside of the prison
