Cannot get water to one toilet???
I have a pipe under it, I have power to the room, all adjacent cells are working perfectly. I've even moved the toilet twice in that room and have dismantled and reassembled my pipe to try to get it to work but nothing is fixing it - continuously blinking water drop on this room. What am I missing here? Bug?

It is in the upper left corner of this building.

I had a big thing written up, but I see your problem now. Small water pipes can only go for 22 squares before you lose water pressure. Time to redesign your water layout scheme.

I suggest 2 simple ways for you to solve this problem:
  • Create an enclosed circuit of smaller water pipes. This should give it more water pressure, and will probably be your cheapest course of action. Or
  • Build some large water pipes heading north from your main line parallel to the right of your electricity, just outside of the prisoner's cells. Then use the small water pipes to sprout off to each toilet.
I personally find the last method to be the most effective with expansion, as well as the most aesthetically pleasing and logically laid out. You won't have to worry about your prisoners tunneling out through the main pipes, as long as there's no large pipes actually inside their cells.
