incident is... nobody knows anymore.
riot is definitely one, or a fire. perhaps a bigger or longer fight?
The only other things that count as incidents, to my knowledge, are riots, murders and escapes. I'd assume fire is one too, but I haven't had one since my first 100 hours or so of game time to know for sure...
The income from no incidents shows what your income on that single day was from having no incidents, the longer your streak, the more money the next day (until you reach the cap, which I can't recall).
As to why your deaths were not counting as an incident I have no idea. I'd assume it is a bug, because they certainly aren't uncommon occurrences these days.
