2 or 3 meals a day?
I only have 2 meal times set up at 11 AM to 1 PM and another from 5 PM to 7 PM

Should I serve breakfast? Is that what they do in real life? I notice some of my prisoners are starving, but I guess they where locked up or in solitary or just busy.

YOU should serve meals, at the minumum before each work session, as this will increase prisoners concentration and effectiveness. I also prefer to feed them at night so they don't wake up quite so crabby.

Also, give them 2 hours for eating, not just 1. will allow them time to actually eat if they're farther away..

I give them 3 one-hour sessions. One after shower before morning work shift, one at midday before afternoon work shift, and another before bed. Everyone seems to get fed just fine (high/high for well-fed bonus) in one hour for me, although in the evening I do give them free time after chow, but it seems like the stragglers are either Sitting or Reading rather than finishing up. Perhaps because I have a relative ton of serving tables - bought them all up front for the entire room capacity, which I don't have enough prisoners to fill yet.

You can also set the quantity and variety in the Policies tab. For 2 daily meal sessions, I go max quantity and regular variety. Or you can coddle them if you prefer, max variety makes them nice and happy but it's expensive. Just keep an eye on the food meter on the Needs page and adjust accordingly.

They get super PO'd without food. When I was completing that dumb nutrition research grant, I had near-constant riots.

Ok, Increase quantity and a bit of variety AND have breakfast and Dinner but no lunch if you're tight for time or budget. That's what I do, so that they have "Well Fed" through their whole work period then they are Well Fed when they sleep and wake up hungry.
