That's probably it. It seems a lot of these prisons weren't made with escape mode in mind; they have all their guards set on either stationary or moving patrols, and there aren't any guards left over to move prisoners. It gets worse if any of the guards die.
I put the game in fast mode and wander out the door in cuffs to do recon on the off-limits areas of the prison. They drag me back to Reception every once in a while, but I head out again to look in a different direction.
Eventually they drag me to a cell.
