I've searched all over and still don't understand
What exactly is wrong with this guy. Are lots of grey plus symbols a bad thing?

The plus symbols indicate the level of strength for any need or status effect. If the need is in green the need is being met and the plus symbols will go down. Generally speaking you should not use one person as a measure of how your prison is doing, but rather hire a Psychologist and then use the "Needs" tab. These + are what help decide if you are satisfying needs. In this picture though: this prisoner has a lower level need to relieve himself, normally means he went recently; his sleep need is green showing he is a sleeping so the + should be going down, he has higher levels of food, recreation, and freedom meaning he has not had this desires met in a while. The drug need shows he has a drug problem. The high suppressed effect suggest either being under heavy guard, or a recent punishment which would slow him from complaining from his lack of needs being met.

In other words alot of grey + = unhappy prisoner more likly to riot.
Green + = that need is being satisied at the moment you are looking at it and should be shrinking.
Alot of + in supression means that the prisoner is unlikly to riot anytime soon (at least until most if not all of the + are gone from supression) also supression has a side effect of making the prisoner have terrible concentration (not a problem if you are not trying to reform them).

All of my prisoners have max suppression grey pluses. I have 3 guards deployed in each cell block and in key corridors, 30 guards in total for 100 prisoners. Could that be the cause?

If they are armed guards then yes that could be causing the supression but if they are normal guards or dog handler guards then no since those 2 don't give off supression. Long punishments can also add supression (lockdown giving out less supression as solitary confinement) and if the punishments are really long then the supression effect will linger for awhile.

In terms of punishment I'm using default, haven't changed anything there. I'm not using any armed guards, just the normal guards only. And I don't have lockdown on the regime.

I know you are using default punishments but are the prisoners getting into trouble alot (which then gets them punished and supressed)?

This is just a guess but I think it's right. I am pretty sure that having 5 plus + will mean that he wants it more than just having 1 +. Also I just noticed that because 2 prisoners just tried to escape and both got tazed and when i leave my mouse obver them the + consistenly go down next to "Tazed"

Yes they are. I get a lot of prisoners in solitary for having tools and recreational items on then. All thanks to metal detectors and a random cell block search I do every 3 days or so. Surprisingly no one is trying to dig their way out yet.

That is probably where your supression is coming from then. Unfortunatly it isn't really avaidable except by changing the punishments or by not letting them into area with weapons but as long as most of their needs are met they wn't try to steal AS much and the ones that do won't make much of an impact on overall pass/fail of programs that you might employ.

They mainly get punished for carrying spoons from the kitchen. 2 hours in solitary. But they keep doing the same over and over.

Speaking of classes. I have 4 classes running all the time. Is that recommended? Does it matter that I only have one classroom? And does classes need to run in work slots on the regime? 3 hours or something?

Well for the spoon punishment you could lower it to lockdown if the supression is too much as long as they arn't digging tunnels and you are removing the supply of spoons hiding in your prison you don't need to worry about it too much.

As for classes there is nothing wrong with having 4 classes going but if you only have 1 class room and you have put in enough work time for all of the classes to run which is 3 hours each... then that means that you have 12 hours of work time.... if you really have that much work time then when do they eat, sleep, or relax? it is also what might be causing alot of desire to steal the spoons in the first place...

Then again I was only assuming that you have 12 hours of work time because you only have 1 class room. If you don't have the time I have specified above then you are not able to run all 4 classes and I suspect you have the "all rooms booked" message appearing for some of them.
One a side note to run multiple classes of the same type it is recommended to have multiple rooms to more effectivly utilyze your regime time other than that good luck!

You're awesome! Thanks for the help. I'll log on and show you my regime and the classes I have got going. Based on everything everyone has said to help I've ironed out most things.

Only if the execution chair really works in the game then I'd execute any prisoner that steals spoons.
