Report a translation problem
I've tried to remove room assignments and re-assign, but that didn't help. Can someone please tell me if I'm missing something? I've been playing this game for quite a while and never had this problem before the last update.

Do any of the in game features cause this problem like infinite money?

My warden will enter his room no problem when it's built, along with other staff. However, those are the only units that will auto assign like they're supposed to.
The warden and staff aren't prisoners. You need to have a fence around the prison to make it secure. Otherwise nobody is going anywhere. If you want help provide the asked for information (aka the screenshot of the whole prison).
Completely get rid of the mod and see if the problem continues. Mods have been breaking a lot of things so you can't address a problem properly without getting rid of it.

I agree with the comments about putting a fence around the whole prison.
I personally have had prisons similar to yours where cell blocks etc. are an integral part of the external wall but its very easy to accidentally end up with a hole in the perimeter (especially during construction work) which causes the problem you are facing and obviously lets prisoners escape if it happens later in the game!
I listed a number of other problems associated with this kind of design but took them out of the post when I realised they were basically spoilers and not related to your specific problem.
Suffice to say, I strongly advise building a basic fence around the entire area left of the garbage/deliveries area before doing anything else and only including 1 large gate as a means of entry/exit. Once this is in place, you can mess around with building positioning and layout as much as you want INSIDE that fence
