Second thing I'd suggest is that you take a look at your prison layout and ensure that there are enough accessible areas (not: gang territory, staff only, or different security classification areas) available to your inmates.
Finally it is possible that one or more mods (if you use mods) are out of date, incompatible with one or more of your other mods, or otherwise broken and are causing this issue. You might need to remove them all and reinstall them one at a time until you find the one that causes the issue to reoccur.
My only major issue now is luxuries. I rarely see any of them visiting the shop, even when I have both min and med security working the shop with an overlap with only medium or minimum security prisoners working while the other has free time. They are always whining about luxuries but seem to be able to tend to their other needs pretty well. I don't want to put a pet bird in every cell (that's reserved for my higher end cells and my supermax guys in permanent lockdown).
Are those working with the latest version? I know the combipack needs an update. I hope Murgh is okay.

LOL, a simple yes, no, or I don't know would have sufficed. Anyway, I grabbed it and it appears to be working correctly. Honestly, I'd forgotten this mod existed.
