Prisoner execution bugging

I have this problem that sometimes when i try to execute an inmate, at step 7 (Escort prisoner to facility) fails and the whole execution is cancelled. I dont understand what i am doing wrong because sometimes it works and sometimes it doesnt.
Has anyone tried this and have a solution?

I have not experienced this problem.
Can you show us an image of the path your death penalty prisoners take from their cell to the execution room. Maybe the issue is somewhere in this area.

Since executions succeded in the past i didnt think it was because of that

Is the Execution room, itself, colored-in with dark grey?
Try that.

PS: If this area is for Death Row inmates only, they do not need a Yard, as they cannot leave their cell. They also do not need a sitting area in the Canteen, so it's OK to make the Canteen small (guards will grab trays of food from the Serving Table and bring the food to each Death Row cell). The only thing Death Row inmates leave their cell for (on their own) is to go to the Parole room.
