"Escort to solitary / padded cell" orders not being created
My misbehaving and criminally insane inmates are not being escorted to solitary / padded cells. Whenever a prisoner misbehaves and should be sent to solitary, he just sits in his normal cell shackled with his "Solitary 2h 00m" timer not ticking down. There is no to-do order indicating that he should be escorted to solitary. No one ever comes to escort him to solitary. I have to manually select him and click on a solitary cell to have him taken there, where the timer will start counting down.

To be clear, there are no staff-only zones blocking access. There's no lack of canteens. "Use cell quality ratings" is turned off. I am not using mods. There are plenty of guards and orderlies. They just won't take anyone to solitary / padded cells unless I manually tell them to do so, one at a time.

Well, jobs engine is screwed for some reason in your save. what shows on your Jobs tab?

What I eventually figured out was that at some point, I only had padded cells for insane prisoners, and did not have padded solitary cells. So the game couldn't figure out what to do with insane inmates who'd committed a "solitary" offense, and stopped creating jobs to do anything at all with them. This also prevented the game from creating jobs to do anything with ANYONE who committed "solitary" offenses, so any time someone misbehaved, they'd just get shackled and forgotten about until I figured out what was going on, changed all of the policies, and cleared out all current punishments.

The way that this game creates tasks and resolves conflicts (ie. doesn't resolve them at all) is abysmal, especially as game systems become a more and more convoluted mess over time instead of being streamlined and made more intuitive.

you're not wrong. the game was badly optimised in that regard from the start, and Paradox's practice of just slapping more DLCs on top of DLCs did... not help
