If you play with gangs on there will always be trouble now and then because when one starts fighting it spreads to the others often.
Snitches etc.. you cant really stop at the start but once you have a section to put protected prisoners in and have the informants in place to be able to identify them quickly you can all but stop that.
And traits like volatile etc.. will always give a bit of trouble.
