Exercise is tricky since some folks are too lazy to do it and it's up to them, but give them yard and yard time, add benches, and those who need it, will be satisfied.
Have a working infirmary for every division of your prison and regularly monitor the Prisoners screen for injuries, make sure they're treated promptly.
Heal drug adiction using reform program. Check often if you have queued prisoners and try to include them.
Sadly there is no known way to heal ALCOHOL addiction.
TL;DR: To give the Health status a fighting chance, feed them well, even as you see them throwing leftovers away and only eating 1/3 of the meal they took to the table.
I personally run 3 meals but only 1h per meal but it's tricky to do well. In the end they will only eat so much. Most of my prisoners stay below 20% in the "time well fed" rating, they oscillate around 15%.
it should work and give them at least 2 health rating points over longer time but best results are if they're fed like this throughout their experience so new inmates after that policy change will benefit the most
Edit: only prisoners who have the exercise need will actually exercise anyway. All prisoners have a 60% chance to spawn with this need, the rest simply will never have the need and thus never exercise.
I did not know that about running, I only get frustrated at them that it takes so long and they don't do other things they should :D but yeah, as I said, some of them are lazy and will never exercise but I still give weight benches to ALL my death row cells ;)
