Health is a tough one, the main thing that drags it down is drug addiction. Other than that, have enough doctors and weight benches.

Health scaled -5 to 5

Percent of stay well fed 1 point per 5%
Percent of stay exercising 1 point per 4% (verification needed)
Percent of stay with untreated injuries -1 point per 1%
Percent of stay intoxicated -2 points per 1%
active addiction -4 points

Well fed= 3 high quality, high quantity meals a day- just in case you didn't know.

Well fed is a status that is given when they eat high variety food that they can take three servings of. I don't think you necessarily need to give it three times a day, but it helps to keep the status "on" for longer times if there are more times it's at least triggered.

Exercise is tricky since some folks are too lazy to do it and it's up to them, but give them yard and yard time, add benches, and those who need it, will be satisfied.

Have a working infirmary for every division of your prison and regularly monitor the Prisoners screen for injuries, make sure they're treated promptly.

Heal drug adiction using reform program. Check often if you have queued prisoners and try to include them.

Sadly there is no known way to heal ALCOHOL addiction.

TL;DR: To give the Health status a fighting chance, feed them well, even as you see them throwing leftovers away and only eating 1/3 of the meal they took to the table.

ok thx everyone for your help. I currently have my prisoners eating twice daily with high variety and high quality meals. Is that enough or do I have to have 3 meals daily?

you should notice some increase and it will probably prevent them from wasting so much food since they WILL be somewhat hungry.

I personally run 3 meals but only 1h per meal but it's tricky to do well. In the end they will only eat so much. Most of my prisoners stay below 20% in the "time well fed" rating, they oscillate around 15%.

it should work and give them at least 2 health rating points over longer time but best results are if they're fed like this throughout their experience so new inmates after that policy change will benefit the most

High variety and high quality gets expensive pretty quickly, I usually dial down the quality to medium, give them 2 hours for breakfast and one for lunch and dinner.

The bonus they get for exercise is badly balanced, but to get the most from it you should really avoid weight benches, instead give them yards to run around in, and yard time too if you wish to force them there. They spend more time exercising by running compared to weight benches, giving a better chance of a bonus to health from exercising.

Edit: only prisoners who have the exercise need will actually exercise anyway. All prisoners have a 60% chance to spawn with this need, the rest simply will never have the need and thus never exercise.

I did not know that about running, I only get frustrated at them that it takes so long and they don't do other things they should :D but yeah, as I said, some of them are lazy and will never exercise but I still give weight benches to ALL my death row cells ;)
