it hasn't happened to me and i play the DLC from the release date. so my guess is something stupid simple is interacting in your game, we'll figure it out

before I give you an answer, those 2 in the Reception, have handcuffs on, so I guess they are for solitary for sure or lockdown. the deployment also seems ok, the staff area in between shouldn't be a problem since, they are "escorted" from reception to the holding cell or the cells.

and as I see your 2 new insanes are still in reception for at least 4 hours waiting for a cell.
i would say since you quick build the rooms, try removing and reassign the room type (padded cells) maybe there is some glitch with quick build......

Do padded cells have grading and is cell grading enabled?
Do psych prisoners use normal cells, or do they require a padded cell?
Will they also use a normal solitary cell for punishments or do they require a padded solitary cell?
psych prisoners can use both types of cells, although it's not recommended, since they can hurt themselves and others within the normal prison.They also get Agitated and Triggered easily in normal cells. For this reason it is highly recommended for the Psych Ward to be an entirely seperate prison away from the normal one.

That's interesting, so they actually lose health when they misbehave in a normal cell?
Would they lose enough health to die in the cell?
It would be interesting of course to see something like that, why not? that's the reason for the padded cells as it is written. it is supposed to be like this.... just as those who don't eat lose health from starvation.
