On 6, yes usually during break time they also go to the security, that room must be bigger than 5x5. I usually build a big security room (5x10? 7x10?) from the beginning because I add the CCTV monitors and phonetaps etc, so I don't have a crowd problem.

For 1, I can say I had an insane that punched the wall and it went down, and then tried to escape. and I couldnt see it because i didnt have an orderly at the area or a cctv camera and also no light on that little corner

this means all types of walls powerstations and objects prisoners should be unable to walkthrough.
Yeah, i've never modified main.dat after the last time i downloaded the game, still after some tests some insane inmates (and sometimes normal too) seems to glitch inside some types of wall as if they get pushed thorough.
